Saúde Criança Renascer

Renascer was founded in 1991 by Dr Vera Cordeiro, as a non-profit organization, with no political or religious affiliation. The constant admission/re-admission cycle of the children treated at the Hospital da Lagoa, a public hospital in Rio de Janeiro - Brazil, was cause for indignation among the health professionals in the hospital. Many children were returning to the hospital after being released, often in more serious condition, some would even die.

Triage

Selection of families eligible for the program according to medical and socio-economic criteria.

If the child has one of the following:

If all of the following apply to the family:

  • Infectious curable illness or malnutrition
  • Serious chronic illness
  • Terminal illness
  • Poverty (income of less than 1 minimum salary per month)
  • No possibility of social mobility
  • Desire to improve

Family Action Plan Definition

The planner (social assistant), together with the person responsible for the family, establishes an initial plan with time periods for carrying out each objective, commitments and respective responsibilities. The Plan is revised periodically and, whenever necessary it is adjusted to ensure the family complies fully with the actions.

Monthly Reviews

Family received food, medicine, psychological/psychiatric and social assistance and legal advice; Lectures and worshops also take place during the family’s monthly visit.
Its progress is also evaluated and if necessary the plan is adjusted.

Discharge

The Socio-medical re-evaluation, the assessment of the families' minimum conditions for self-sustainability and the achievement of the objectives established in the Family Action Plan determine the conclusion of the program. Average involvement is usually two years.

Family Action Plan (FAP)

The Family Action Plan is a plan tailored to each family.  It encompasses 5 areas:

Health - Renascer provides food, medicine, medical equipment, diapers, condoms, psychological, nutritional and
social assistance and recreation for children at the hospital.

Income - Renascer promotes vocational training courses and work tools so that families can generate/increase
their income and achieve self-sufficiency.

Housing - Renascer refurbishes houses when necessary, providing the materials and labor.

Education - Renascer offers workshops and lectures on topics such as: family planning, nutrition, sexual transmitted diseases, pregnancy during adolescence etc.

Citizenship - Renascer provides legal advice.  It assists families in updating their documentation and in getting social benefits.
